ATFL Ankle Sprain

Comprehensive diagnosis, symptoms, causes, and treatment options

What is ATFL Ankle Sprain?

(Anterior Talofibular Ligament Injury)

An ATFL ankle sprain is the most common ligament injury of the ankle, occurring when the anterior talofibular ligament is overstretched or torn. Though often perceived as minor, inadequate treatment can lead to chronic ankle instability and recurrent sprains. Early, expert care ensures complete recovery and long-term joint protection.

Symptoms

  • Sudden pain on the outer side of the ankle
  • Swelling and bruising around the ankle
  • Tenderness over the front-lateral ankle
  • Difficulty walking or bearing weight
  • A feeling of instability or giving way

Causes

  • Inward rolling (inversion) of the ankle
  • Sports-related twisting injuries
  • Walking or running on uneven surfaces
  • Poor footwear or inadequate ankle support
  • Previous ankle sprains leading to ligament weakness

Treatment Options

Treatment is tailored to the severity of the ligament injury.

Non-Surgical Management

RICE protocol (rest, ice, compression, elevation)
Functional bracing and early mobilization
Targeted physiotherapy for strength and balance
Gradual return to activity under supervision

Surgical Solutions

Surgery is reserved for:

Chronic ankle instability
Recurrent sprains despite rehabilitation
High-demand athletes requiring ligament reconstruction
